I need our forms to go to three different email addresses. One of the email addresses, no matter how many times I add it to the recipient list, is not receiving the form submissions. Can you help me out?

I have checked the email addresses that you're using in your form http://www.jotform.us/form/42884799504168 and I can see one of them is in our bounce list.
Reason(s) : reject
I have removed it from our bounce list and you should receive emails on that address now.
When checking your form I noticed you have some missing info such as the "Reply-to" email address, please do note that some email providers with missing info will not accept emails, I would suggest you to check this guide in order to know how to properly set up your email notifications and avoid bouncing: https://www.jotform.com/help/208-How-to-Prevent-Email-Bouncing-Related-Issues
