I want a form where a artist can download a form and sign and send back to me.

You can convert your form into a fillable PDF and send this PDF file to your users. Here's a guide: How-to-Create-a-PDF-Form.

You may send it via email or by uploading it to your web server. After that, you can create a link that will allow your users to download the fillable PDF form. If you want to create a downloadable link in your form, please check this guide: How-to-Add-a-Downloadable-File-to-Your-Form.

Once the fillable PDF form is downloaded, they can just open the file in a PDF reader or program. After filling up the form, they need to save it and then send the file back to you via email.
