How can I set up conditional Option for Thank You Page

    My last survey question is whether the person wants to leave their name and email address.  I'd like to send them to one TY page if they do leave their email and another if they do not (so if the field collecting email is blank).

    1. Is it possible to direct to two different email addresses based on a question answer?

    2. If yes, in 4th grade level terms, can you tell me how to do that please.

    3. On one of the thank you pages, I would like like to add text directing them to a link to register for an appointment with me. Possible to embed a link and if yes, how?

    I will answer your queries one by one.

    Based on your query, you can add a condition in your form. Go to Settings > Conditions > Change Thank you page.

    When an email field is filled, the condition will be set up as explained below. You can insert a link into your Thank you page also as explained in the screenshot below.

    In a similar way, you can set up conditions if the email field is blank.

     Yes, it is possible by using the "Change E-mail recipient" condition. But before setting this up, you will have to set up the Email notification - Setting-up-Email-Notifications
