Is it possible to make an order confirmation form in one way or another?

The intention is that we feed the form with details of the customer and the order and that the customer can then confirm this (make a choice between options) and sign.

I have tried different templates but I do not find that we can feed first and then send it to the customer to have it completed and signed.

You can achieve this using the Edit Link in the Email. You can use this feature to send/received confirmation email (Notification), while the customer will get the Autoresponder Email.
