Queries while converting to a fillable form

    Hello,

     
    Can you please let me know the cost to make this PDF into a fillable form? 
     
    Where the user fills out the form from a website and the owner of the website receives the PDF fill out with the client information?

    There are no charges for making the PDF into a fillable form.

    To create a fillable form, you can import your fillable PDF form into JotForm. This guide will help you out: How-Can-I-Import-My-Own-PDF-to-JotForm.

    Do note that this feature will not accept plain PDF files or documents. It has to be a fillable PDF and needs to have fillable form fields (e.g. textboxes, text areas, checkboxes, radio buttons, etc.).

    Unfortunately, the empty PDF does not have an embed code. Only the online form can be embedded on a website.

    Fillable PDF forms only work properly when using a program such as:

    Adobe Reader: https://helpx.adobe.com/acrobat/kb/acrobat-downloads.html 

    Or Foxit: https://www.foxitsoftware.com/products/pdf-reader/ 

    If you would like to embed your form to a webpage to be used online, I would recommend using the web form: Which Form Embed Code Should I Use.

    Once that is done, you can set up email notifications in the form so that the owner receives the customer info - Setting-up-Email-Notifications
