I am not able to sign in and my forms are disabled.

    I dont understand why I cant sign in or why my forms are disabled.

    I did check your account and found that it is currently suspended as your forms are suspected to be used for phishing.

    I am escalating this thread to our form review team. We will get back to you as soon as we have any update from them.

    It seems your account was suspended because of the "Driver's license #" field you have added in form "RENTAL APPLICATION".

    You may continue to collect that information, however you must add an agreement displayed on the form and a signature, widget fields. You may like to take a look at the following guide on how to add widgets to your forms: https://www.jotform.com/help/252-How-to-Add-a-Widget-to-Your-Form
