Show text on form when printed

Hello,

I am considering upgrading once I have the form working.

I have two questions please:

1/ On this form:

I would like the consent info:

"I, the undersigned, declares"

to show up on the printed form, for legal reasons I would like the question and answer to show up on the form results, by email and when printed.

2/ What are the best image uploaders available.

For the email notification, you can manually add the content to the notification email content template.
