Creating a Tax Return Checklist

Are you able to suggest a template or the best way to create an online form that:

• asks a client questions that correlate to a tax return
• collective receipts and other supporting files (eg spreadsheet, PDFs) for each applicable questions
• save that information to Dropbox or OneDrive
• be embedded in a website or attached to an email
• has our firm’s logo on it
