Adding Upload File option!

I have a question, I want to add an option to upload/Attached document into form.

Is Jot form give us option to do this ?

To clarify, do you want to create multiple forms and show the forms based on the user's choice?

If so, then you may create a main form and embed the other forms in it using iframe embed widget.

Then, add conditions like the ones below to show selected forms accordingly.
