    Hello,

    If I'm not mistaken, you would want your form to appear on a web page. If so, there are a number of options on how to make your form appear in them. You can check this guide for more information: https://www.jotform.com/help/67-Which-form-embed-code-should-I-use

    Now with regarding to how it should be used: For every submission that you receive, they land into your account and in which these submissions can be viewed into your form's submissions page. You can check this guide for more information on how to view your submissions: https://www.jotform.com/help/269-How-to-View-Form-Submissions

    You can also change your thank you page (the text that's showing after making a submission), to either a text or be redirected to a different page: https://www.jotform.com/help/211-Setting-Up-the-Thank-You-Page
