Form emailing and showing wrong information

Hi -

When our user submit our form it does not get linked with the appropriate IEA 2017 team.  When the form is submitted it gets the message that it is linked to an Arsenal (soccer team) and all of the colors say red and black. Somehow our order site is linked on the back end to the wrong team.  How can I fix this?

The email settings seem to be outdated on your form.

I would like to suggest:

1) Removing the current email notifications and autoresponder.

2) Adding the new email notification (usually emailed to the form administrators) and autoresponder (usually emailed to the form submitters).

3) Also, check the form Thank You Page settings to make sure that you have a correct information there.
