Can I send a completed form on only for signature?

    I have designed a form which allows me to enter all the details for courses which I provide the training for.  I would like to enter this detail, changing these fields to read only, but send this on to the company rep booking me to sign.  Please help :)

    To clarify, do you want to disable or set the fields read-only after you fill out the form? If so, you can use conditional logic to disable the field, though you need a field to trigger that condition.

    Guide: How-to-Enable-Disable-Fields-Using-Conditional-Logic

    I'm assuming you will send the form on the company using the edit link of the form? If so, you can enable certain fields using that conditional logic as well. However, to trigger the condition in form edit mode, you will need to use a "GET FORM PAGE URL" widget.

    Then use a condition like this:
    If "Get form page URL" contains "edit" then enable (fields)

    Hi, thanks for your answer, and apologies for the confusion!


    Yes, ideally I would like to fill in the form with the necessary detail, then somehow lock those answers in (so the end user cant change them).  I would then like to send a link for the end user to sign the document, and add their name, position and date.

    If that is the case, you may add the E-signature widget on your form, then create conditions to show it on edit link only: https://www.jotform.com/help/389-How-to-Show-or-Hide-Fields-on-Edit-URL

    And also, create conditions to disable the rest of the fields on edit link only, example:

    Is it only one rep who will sign the submissions? If that is the case, simply create an notification where you should put the rep's email address as the recipient: https://www.jotform.com/help/25-Setting-up-Email-Notifications

    Note: there are two incomplete conditions, please complete or delete them to avoid issues in the form:

    Hi,

    I really am still struggling!

    To clarify what is it that I am after; I would like to fill in the detail on the course detail form, and then for the form to be sent on for the bottom four questions to be signed.  How do I complete the data for it then to be sent on? Will they get to see it with all the formatting?

    Following the email link, they get a breakdown of the empty cells, without any of my added detail and without the ability to sign or edit?


    Please, please help! I have just upgraded my account for further options but I'm still really struggling!

    We can help you achieve your requirements, or if you want we can modify your form based on your requirements? Just give us permission to edit your form so we can apply the necessary changes. Don't worry, we'll explain the methods so you can learn it too.

    Please answer these questions first:

    1.) How you will send an email to the backend user? By autoresponder or notification?

    If autoresponder then we will use an email field and then send the autoresponder to the email entered in the email field.

    1. Which would you recommend? I'm not fussed to be honest! Does the user have to be a jotform user to be a notification?

    2. The end user should only be completing;

    Signed by:

    Full Name:

    Position within company:

    Date completed:

    I suggest using autoresponder instead of notification if you will send the form to different recipients.

Also, the signature field can't be conditionally disabled so I hid it instead and then it will only show if the form is in edit mode.

    Sorry about that, it seems that the update I made to the autoresponder was not saved successfully. Kindly check and try the form again.

    Hi! That is working and the email has come through perfectly for signature! 

    Few things - the ‘position at company’ field doesn’t appear? 

    Also on the email I received after the signature appears but the other fields (full name, date of completion etc) doesn’t?

    There seems to be an issue with the first "position at company" field so I replaced it with a new one. Also, the only fields that can be changed are these (please check my screenshot below):
