What are the ways to make a unique identifier for forms and patients?

    We are sending diagnostic kits by mail and the users will register the kits using a Jotform when they are ready to use them. The data from the form will then be used to populate a database in Active Campaign which Jotform has the API for. We'd like to see if we could do the following using some sort of combination of unique identifier and UPC widgets.

    1) We were wondering if there was a widget that we could put on each form so that each kit would receive a unique code that could then be tied to the person who registers the kit by filling out the Jotform. 

    For instance, when the registrant opens up the form it will be given a unique UPC code that he or she can simply write on the return test kit, which then ties what was filled out with who filled it out with the kit it was filled out for. We can then ultimately add the results back to the database using the UPC code and registrant info. 

    2) The other thing we would like to find out is if it would actually be possible to almost anonomyze the registrant and assign her or him a number which is associated with their name. We would then associate everything related to that patient with a non-identifiable number. I'm sure you've seen the same thing with anything associated with signing into your hospital or insurance account.

    You can use the Unique ID field for this. Check the user guide: How to Add Custom Unique IDs to your Form Submissions?


    For #2

    You can use the Unique Question feature of the form to create a unique field that you can associate to another data from your form.

    Thanks Jonathan, this seems just what I need for #2. Maybe you could help me get it exactly right - I'm still a little lost. My Jotform has sections for the registrant's mail address and birthday. I want to use the combination of the two to generate a USER NAME/NUMBER and I've added a short text box to the jot form to place the USER NAME/NUMBER that will be generated. 

    I'm using the Condition's routine. A) I've chosen the first IF statement to be the USER'S EMAIL and the STATE to be FILLED and B) I've chosen the second IF statement to be the USER'S DATE OF BIRTH and the STATE to be FILLED. 

    I next chose the statement IF "ALL" of the rules are matched DO CALCULATE A FIELD'S VALUE.

    But this is where I get lost. How do I get the two Conditions to calculate a unique USER NUMBER that can then populate the USER NAME/NUMBER text box. Needless to say, I would prefer that the USER see his or her USER NAME/NUMBER. 

    I'd also like to add an integration that would mail the USER NAME/NUMBER to the patient using his or her email and a template email that will populate with the correct first name.

    The condition inside of your form is set properly. You just need to change the field values to String (A-Z).

    Very neat. Thank you so very much, this is close to perfect. I have another question. The string that is generated could be something like this "February 9 1957drq@cignaturehealth.com" which isn't as elegant or in the form as I'd like. Is it possible to convert the string into a series of numbers or numbers and letters without the spaces and symbols like "@" and ".". I'd like to use the chosen fields to establish a USERNUMBER because they are unique, but don't actually want it to be simply the combination of the two verbatim. I would, instead, like it to be a unique alpha numeric that depends of the combination.

    Unfortunately, that is not possible.

    Beside the Unique ID widget that my colleague suggested, we also have the Random Value Generator widget. Maybe you can use this widget or combine this widget with some other form field to create a USERNUMBER.

    Gave it a try and it was very interesting and now getting me to rethink what we can do.

    I can use the Random Number Generator to simply create a 7 digit alpha-numeric code and use it as the USERNAME.

    The next thing I'd like to do is have that USERNAME automatically put on every form and report that the USER gets so we essentially have all of her or his forms, reports, purchases, etc. in one folder that belongs to the USER or at least know what belongs to who. 

    How do I do this? I can keep it simple by putting the field on each of the forms and having the patient put it in. But since the USERNAME can be rather hard to remember, I can key everything to their email and then, if at all possible, have a box that would pull the USERNAME down to the form. 

    It seems something like the earlier suggestions with a slight twist. Could you perhaps point us in the right direction?

    Do note that using the random generator, every time the form is submitted a new random number is generated.

    Unfortunately , there is no feature to automatically attach a unique number on every form or report of a user.

    You can however, download the submissions and sort it by using reports or excel.

    Thank you for letting me know that. I still can use the generator for our purpose since each individual will only use the form once at registration. I'm only interested in each getting a unique number when they use the form that can then essentially be used as their USERNAME moving forward.

    I simply want to know A) if the submitted form will have the randomly generated unique number in the CSV file so we know what it is and B) if there is a way to capture the number so that I could send it in the THANK YOU message or a confirmation email after they hit the submit button that would then say something like "Thank you for registering. Your USERNAME IS AX43278. Please write it down and save it to log into the system and register your kit when it arrives." This is essentially what is said at the top of the page too.

    You will be able to see and download all randomly generated unique numbers from the submissions page of your form.

    You can display the Unique number to your users on the submission page and send them the same via an Autoresponder Email.

    I would also suggest you to hide that field from your form. The Unique code will be saved only when the form is submitted.
