Hallo . ich hätte eine Frage , für einen Aufnahmebogen für meine Patienten habe ich folgende Anforderungen:

Einlesen von Daten aus eine GDT Schnittstelle aus meinem Administrationsprogramm

elektronische Unterschrift

Versand des Formulars per PDF oder einlesen in das Administationsprogramm SQL based

Sien diese Optionen mit IHrer Software möglich ?

Once your form is submitted, it can automatically generate a PDF file and send it in email:

I have some more questions:
I would like to create a “hello , welcome” form for my patients for collecting an up dating personal datas, eg. address, current medikation and so on.
1. Is it possible to create a database with the data filled out on the Forms, so the following forms for each patient can automaticly show the last data from the form the patient filled out eg 6 Month ago , so that the patient only need to make some corrections and the signature?

Hi Michael, 

To avoid any confusion, let me answer your questions separately. 

1. Is it possible to create a database with the data filled out on the Forms, so the following forms for each patient can automatically show the last data from the form the patient filled out eg 6 Month ago , so that the patient only need to make some corrections and the signature?

Here are some options that I can suggest to consider. 

First, you can attach a database of your patients to form through Spreadsheet to Form widget. This widget can populate form fields based on unique id entered. Let's say, a user can enter their email address and form will populate fields for that patient.

The other suggestion would be to generate unique links for users (the links would prefill a form via URL parameters). However, this depends on the amount of data to hold, if you have a big form, maybe first option would work better.
