One to One Health discovered Jotform Enterprise when searching for a solution to support the Tennessee Valley Authority’s response to COVID-19. Since then, the organization has expanded its use of Jotform Enterprise across several processes and clients.

Demonstrating value and return on investment with Jotform Enterprise

When One to One Health launched its virtual primary care product, Text Care, it attracted 19,000 users in roughly one year. Using Jotform Enterprise, One to One Health asked patients if they liked the service, what they would have done had it not been available, and in which medium they preferred talking to healthcare providers.

”Sometimes people ask, ‘Do people want a text about their healthcare?’ So we asked and ranked the responses to demonstrate preferences,” said Kinzler. “In fact, people prefer text messages by 94.6 percent.”

In addition to showing demand for its services, One to One Health uses the reporting in Jotform Tables to demonstrate its clients’ return on investment.

“I can tell employers, ‘Based on patient-reported data, 33 percent of Text Care patients would’ve gone to urgent care, and 5 percent would have gone to the emergency room,’ ” says Kinzler. “Because there are costs associated with those facilities, we can show employers how the service they are paying for benefits them and the amount of money saved.”

One to One Health also maximizes qualitative data by tying patient feedback to employers for personalized insights and customized reports, including Net Promoter Scores® (NPS®).

“Our NPS is 96.5, which sounds unbelievable,” he says. “But I’ve anonymized the data so I can show it. It’s super powerful to prove our value to skeptical buyers.” Staff can also sort scores by employer to calculate NPS scores for each client.

Improving customer experiences and response rates with Jotform Enterprise

Previously, One to One Health relied on whichever third-party provider integrated with its Electronic Medical Records (EMR) system to administer patient satisfaction surveys. The service was expensive and offered no autonomy to customize questions, which resulted in lower response rates.

“I couldn’t ask patient-specific questions or remove irrelevant questions because the survey was hard-coded into the EMR system,” says Kinzler. Now, using Jotform Enterprise, One to One Health can personalize the outgoing message so it appears to come from the patient’s provider, reinforce trust and credibility by branding the survey to match its website, customize the surveys for each client, and pull the data for analysis and reporting.

Driving efficiency and enhancing care with Jotform Teams 

Often, providers spend the first 10 minutes of an appointment waiting for the patient to fill out paperwork to see where that individual is and how they have progressed.

The Patient Health Questionnaire (PHQ-9) and Generalized Anxiety Disorder (GAD-7) surveys in One to One Health’s EMR system had dynamic form links based on the patient’s chart, making them time-consuming to send out. In addition, submission data exported as a PDF inside the patient’s chart, making aggregate tracking impossible.

Now the Behavioral Health team uses Jotform Teams to manage forms and automate mental health services. Tillman Endsley, VP of strategy and operations, recreated a mobile-friendly version of the form that’s emailed to patients 15 minutes before their initial conversation with their provider. In addition to enabling form filling on any device, One to One Health can send the submission data directly to the relevant provider to track patient progress.

Sending out the forms is faster and easier, and instead of manually transcribing the data from the EMR, One to One Health uses Joform’s API keys to feed the data to client records.
