Use this completion report form if you are hosting an event at the school and the administration requires a completion report for them to assess the outcome such as if the event achieves its purpose and if students learned something about it.     One report per-quarter can be filled out for recurring events or campaigns (e.g. brown-bags, volunteer fridays); adapt the report as needed to include as much detail as possible.
