Template For Quiz

This is for template for a 10 question True/False Quiz. I have left it set up with the certificate that I used to send a certificate out to anyone who did the training and quiz. It is set up to send out the certificate to the student and our human resources department. I also have it set up to email the graded quiz to the human resources department. For that, I set the pass rate to be 70%. I designed a form in Word for the certificate and left space for the Name of the student and the Date. I then saved that form as a pdf and converted it to a png file and uploaded it to the background so that it would automatically pull the name and date fields in. I learned the hard that that every time I updated the form of the test, I would have to update in the pdf editor to change the quiz print out. Two fields have to be tweaked to delete the words "name" and "date" on the certificate to print correctly and it will pull in all of the questions and those fields have to be deleted every time you update the form--not with every submission unless you go into the pdf editor, it will send the proper certificate with the name of the training class. When you edit the template to enter your own questions, you will need to click on the gear icon to enter the correct response which is indicated with a zero (0) or a one (1) towards the middle of the second option tab. I set it up to figure out the percentage rather than the number of correct answers. If you change the number of questions, you will need to change the math which is a higher level of brain power than I wanted to use. Hope this helps and that you enjoy it. I am new to Jotform and this was a big learning curve for me. With the help of the support team, I was able to come up with a form that is easy to use, easy to edit, easy to share information. Because we are a healthcare agency, it is also hipaa protected which added a challenge with having to set a password. There is really no HIPAA information in these forms but I wasn't able to turn that option off. If you don't have to use a hipaa platform, you can choose not to password protect them which would make sense. If you are on a HIPAA platform, hopefully you can just change the password during the editing to one that you or your employees will use to open the attachments. Happy JotForming!
