What are self evaluation forms?
As its name implies, it is a type of performance evaluation forms, only to be conducted by the actual employees themselves. Certain questions are asked to employees in these forms, both linear scale and open ended form, and employees are expected to evaluate themselves as objectively as possible.
Why are self evaluation forms important for organizations?
Employee performance appraisal is always important in order to improve the performance of the team along with the performance of the individuals. It also helps to monitor the progress of the employees and the performance of the individual team. These evaluations are quite useful to identify the professional development areas, strength and weaknesses of an employee that can be improved.

Which questions should be asked in self evaluation templates?
Sticking to deadlines: The employees should evaluate themselves with their ability to make the deadlines for a given task or project.
Communication skills: Not surprisingly, having strong communication skills is expected from virtually all the employees within a company. So it should be included in the self evaluation process as well.
Proactiveness/Reactiveness: They are not preferable one over another necessarily, although being proactive usually does not hurt. This question gives employees the chance to explicitly say that they need to be more proactive.
Improvement areas: This is another cliché of performance assessments, but it is for a reason. Employees with high level of self-awareness can make good calls on themselves.
