    Eligible candidates may apply by sending the duly filled application form as e-mail attachment to life.harvest.training@gmail.com on or before 08.20.2017. Interview date & time will be informed to the shortlisted candidates in due course by e-mail. No applicant will be paid for attending interview or for joining, if selected. Candidates will be required to produce certificates/testimonials in original and one set of attested photocopies at the time of interview.

  • PHASE I: INTERNSHIP SUMMARY

    Phase 1 Duration: 16-weeks

    Weekly Commitment: 6-10 hours

    Objective: Designed to strengthen your idenity (who you are), bring clarity to your vision, and provide boots-on-the-ground leadership experience to young and seasoned leaders, affordably through cooperation;

    Evaluated on an individual basis, throughout phase I, interns will be shared cooperatively across one or multiple established marketplace ministries or afflliates, maximizing the intern's teaching, training, and equipping received.

    A short summary of what you will learn:

    1. How to research qualified and busy business contacts from LinkedIn
    2. How to archive said businessmen in a Google Docs Spreadsheet and share with leadership
    3. How to move a vetted contact through a systematic communication process
      How to send scripted introductory e-mail, follow-up with a phone call, and schedule a 15-minute coffee
    4. How to send handwritten thank you notes
    5. How to represent our marketplace ministry(ies) in meetings and industry events
    6. Build some healthy systems and processes and methods by which to reach the millennial audience

    Preferred skills:

    • Prior experience in a fast-growing tech environment a plus
    • Keen analytical skills and a creative problem-solving approach
    • Exceptional customer service skills
    • Our ideal candidate is approachable, trustworthy, professional, and shows integrity.

  • INTERNSHIP AGREEMENT
    Read this agreement and then mark at the bottom. You are entitled to a copy of this agreement.

    Life Harvest, LLC (LH), Mayfield Ministries (MM), and direct partnering organizations.

    1. Upon acceptance into its internship program, the status of the intern with LH, will be as outlined for trainees in the Fair Labor Standards Act and nothing in this agreement or subsequent relationship shall be in any way a violation of that act.
    2. LH may at any time in its sole discretion terminate this agreement without notice or cause.
    3. The education received by the intern is for the benefit of the intern.
    4. The intern will not replace or displace regular employees of LH.
    5. The training received, even though it includes actual operation of the facilities of LH, is similar to that which would be given in a vocational school.
    6. LH does not receive an immediate advantage from the activities of the intern.
    7. LH in no way through direct statement or interference guarantees employment to the intern during, or at the conclusion of the internship program as a result of the training received or otherwise.
    8. All parties involved understand that that intern is NOT ENTITLED TO WAGES or other payments for the time spent in training.
    9. LH assumes none of the “ex loco parentis” responsibilities of any school, institute, or other educational organization with respect to the intern.
    10. All rules and regulations governing LH employees shall apply equally to all interns and all interns will be provided with these rules and regulations either orally or in writing.
    11. It is agreed among the parties that the intern while on the premises of LH, MM, and its direct partners or in their involvement in connection with the organization is not to be considered an employee or agent of LH, MM, and its direct partners for any purposes, including but not limited to workers compensation. The intern is at all times to be considered a employee or student of the entity sponsoring this intern and doing assigned work as part of his/her curriculum/training.
    12. The sponsoring entity (organization/school) will at all times indemnify and hold LH, MM, and its direct partners harmless from and against any claim, demand and liability whatsoever that LH, MM, and its direct partners may sustain or incur by reason of or as a consequence of this internship.

    The term of this internship is determined by the sponsoring entity,independent intern, and LH, MM, and its direct partners, which begins and ends as set in the application or immediately after LH's receival.

  • MARKETING WAIVER

    I give Stokes Mayfield's affiliated partners, partners, sponsors, Mayfield Ministries, Life Harvest, LLC, Life Harvest Internships, Kicks 'N' Cones (herein, “Life Harvest”), J. Stokes Mayfield, Jr., Board of Advisors of Life Harvest and Stokes Mayfield, Jr., Board of Influence of Life Harvest and Stokes Mayfield, Jr., Board of Directors of Life Harvest and Stokes Mayfield, Jr., its employees, designees, agents, independent contractors, legal representatives, successors and assigns, and all persons or departments for whom or through whom it is acting, the absolute right and unrestricted permission to take, use my name, testimonial and/or publish, reproduce, edit, exhibit, project, display and/or copyright photographic images or pictures of me or my child(ren), whether still, single, multiple, or moving, or in which I (they) may be included in whole or in part, in color or otherwise, through any form of media (print, digital, electronic, broadcast or otherwise) for art, advertising, recruitment, marketing, fund raising, publicity, archival, or any other lawful purpose. I waive any right that I may have to inspect and approve the finished product that may be used or to which it may be applied now and/or in the future, whether that use is known to me or my child(ren) or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image or product. I release and agree to hold harmless Stokes Mayfield's affiliated partners, partners, sponsors, Mayfield Ministries, Life Harvest, LLC, Life Harvest Internships, Kicks 'N' Cones (herein, “Life Harvest”), J. Stokes Mayfield, Jr., Board of Advisors of Life Harvest and Stokes Mayfield, Jr., Board of Influence of Life Harvest and Stokes Mayfield, Jr., Board of Directors of Life Harvest and Stokes Mayfield, Jr., its employees, designees, agents, independent contractors, legal representatives, successors and assigns, and all persons or departments for whom or through whom it is acting, of and from any liability by virtue of taking of the pictures or using the testimonial/biographical data, in any processing tending towards the completion of the finished product, and/or any use whatsoever of such pictures or products, whether intentional or otherwise. I certify that I am at least 18 years of age (or if under 18 years of age, that I am joined herein by my parent or legal guardian) and that this release is signed voluntarily, under no duress, and without expectation of compensation in any form now or in the future.
  • WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT

    1. In consideration for receiving permission to participate in the internship, I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE Stokes Mayfield's affiliated partners, partners, sponsors, Mayfield Ministries, Life Harvest, LLC, Life Harvest Internships, Kicks 'N' Cones (herein, “Life Harvest”), J. Stokes Mayfield, Jr., Board of Advisors of Life Harvest and Stokes Mayfield, Jr., Board of Influence of Life Harvest and Stokes Mayfield, Jr., Board of Directors of Life Harvest and Stokes Mayfield, Jr., its employees, designees, agents, independent contractors, legal representatives, successors and assigns, and all persons or departments for whom or through whom it is acting, (hereinafter referred to as RELEASEES) from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in such activity, while in, on or upon the premises where the activities are being conducted, REGARDLESS OF WHETHER SUCH LOSS IS CAUSED BY THE NEGLIGENCE OF THE RELEASEES, or otherwise and regardless of whether such liability arises in tort, contract, strict liability, or otherwise, to the fullest extent allowed by law. 2. I am fully aware of the risks and hazards connected with the activities of Martial Arts Confidence Courses, and I am aware that such activities include the risk of injury and even death, and I hereby elect to voluntarily participate in said activities, knowing that the activities may be hazardous to my property and me. I understand that Stokes Mayfield's affiliated partners, partners, sponsors, Mayfield Ministries, Life Harvest, LLC, Life Harvest Internships, Kicks 'N' Cones (herein, “Life Harvest”), J. Stokes Mayfield, Jr., Board of Advisors of Life Harvest and Stokes Mayfield, Jr., Board of Influence of Life Harvest and Stokes Mayfield, Jr., Board of Directors of Life Harvest and Stokes Mayfield, Jr., its employees, designees, agents, independent contractors, legal representatives, successors and assigns, and all persons or departments for whom or through whom it is acting, does not require me to participate in this activity. I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury, including death, that may be sustained by me, or any loss or damage to property owned by me, as a result of being engaged in such an activities, WHETHER CAUSED BY THE NEGLIGENCE OF RELEASEES or otherwise, to the fullest extent allowed by law 3. I further hereby AGREE TO INDEMNIFY AND HOLD HARMLESS the RELEASEES from any loss, liability, damage, or costs, including court costs and attorneys' fees that Releases may incur due to my participation in said activities, WHETHER CAUSED BY NEGLIGENCE OF RELEASEES or otherwise, to the fullest extent allowed by law. 4. It is my express intent that this Waiver and Hold Harmless Agreement shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representative, if I am deceased, and shall be deemed as a RELEASE, WAIVER, DISCHARGE, AND COVENANT NOT TO SUE the above-named RELEASEES. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be construed in accordance with the laws of North Carolina, South Carolina, and the State of Georgia and that any mediation, suit, or other proceeding must be filed or entered into only in Georgia and the federal or state courts of Georgia. Any portion of this document deemed unlawful or unenforceable is severable and shall be stricken without any effect on the enforceability of the remaining provisions. IN SIGNING THIS AGREEMENT, I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing Wavier of Liability and Hold Harmless Agreement, understand it and sign it voluntarily as my own free act and deed; no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made; I am at least eighteen (18) years of age and fully competent; and I execute this Agreement for full, adequate and complete consideration fully intending to be bound by same.
  • CONFIDENTIALITY AGREEMENT

    This letter is a reminder that any nonpublic information acquired, in whatever manner and from whatever source, in the course of service for, to or on behalf of Life Harvest, LLC (LH), Mayfield Ministries (MM), its direct partners, and its affiliates and subsidiaries (collectively the “Company”) is “Confidential Information” owned solely and exclusively by the Company, and is valuable, unique, proprietary and confidential. Confidential Information includes, but is not limited to, all information relating to the Company’ past, present or future business activities, pricing, research, product design or development, business opportunities, personnel and any information disclosed in confidence. Your obligation to maintain confidentiality may also be required by you or your company’s execution of a nondisclosure agreement or other agreement containing confidentiality provisions.All confidential and proprietary information must be held in confidence and not disclosed to any other party. Any disclosure could result in irreparable harm or significant injury to the Company for which money damages will not adequately remedy. Upon termination of any services for the Company, all written or descriptive materials and documents containing any confidential information along with copies or derivations, including but not limited to machine-readable documents, must be returned or destroyed.THE COMPANY TAKES SERIOUSLY ANY BREACH OF CONFIDENTIALITY, AND WILL TAKE ALL APPROPRIATE ACTIONS TO ENFORCE THE OBLIGATIONS DESCRIBED HEREIN.
